Job Description

DSHS Maple Lane – Forms & Records Analyst 2
Maple Lane Behavioral Health and Treatment Center in Grand Mound, WA is seeking a Forms & Records Analyst 2 to join our team in a vital role that supports legal and operational transparency. This position is responsible for managing and coordinating responses to subpoenas, discovery requests, and public disclosure inquiries from a wide range of sources including attorneys, courts, government agencies, and media outlets. The analyst ensures timely and consistent retrieval of records, applies appropriate redactions, and follows established procedures to meet legal and policy standards. Additionally, the role oversees records retention, transfer, migration, and disposition using both manual and electronic systems, helping to maintain the integrity and accessibility of critical documentation across the organization.

   

Some of what you'll do:
  • Process and respond to legal subpoenas, discovery requests, and public records requests under the guidance of designated leadership.
  • Review and track requests to determine scope, required information, and completion status.
  • Coordinate with departments to gather records, correspondence, and other materials from various sources, including electronic and hard copy files.
  • Identify and manage requests requiring special handling, such as court-ordered redactions.
  • Ensure records are organized, maintained, and protected in compliance with retention schedules and policies.
  • Assist with records inventories, archival preparation, and proper disposal of materials.
  • Collaborate with legal counsel and requesters as needed to clarify and complete requests.

Desired skills, knowledge, and abilities:
  • Knowledge of records and forms management practices, including state and federal rules governing clinical, medical, legal, and administrative records.
  • Understanding of the Health Insurance Portability and Accountability Act (HIPAA) Privacy and Security Rules.
  • Familiarity with Washington State court rules related to discovery, subpoenas, and the production of materials.
  • Proficiency in Microsoft Office and other computer applications used to scan, convert, archive, and transfer records.
  • Strong organizational skills with the ability to manage multiple tasks, set priorities, and meet deadlines.
  • Ability to work independently with minimal supervision while maintaining attention to detail and accuracy.
  • Effective communication and interpersonal skills to collaborate with staff, legal counsel, and external requesters.

Who should apply?
We are looking for professionals who meet one of the following qualifications:
  • An Associate’s degree in business administration, accounting, or a related field
    OR
  • A high school diploma or GED and at least three years of experience in forms and/or records management.

Additional information:
  • Work schedule: 8:00 AM - 4:30 PM, Monday through Friday
  • Opportunities performed on-site at this work location at least three days a week may be eligible for an additional 5% premium pay in addition to the advertised salary.
